内容摘要C型利尿钠肽对小鼠Cajal间质细胞起搏电流的影响1893年，西班牙神经解剖学家Cajal用甲基蓝及嗜银染色法在胃肠道神经系统entericnervoussystem，ENS中观察到一类特殊的间质细胞，命名为Cajal间质细胞interstitialcellsofCajal，ICC。ICC是ENS中的一种非神经但与神经有关的特殊间质细胞，胃肠道平滑肌的运动形式在很大程度上取决于ICC的活动。已有众多研究表明，许多胃肠道平滑肌的静息电位不稳定，表现为缓慢的起伏波动，即周期性去极化和复极化，称为慢波slowwave，SW，也称基本电节律BasicElectricRhythm，BER。BER决定着平滑肌收缩节律，调控着胃肠道运动发生的时间、地点、频率、和方向。近年来的单细胞记录和分子水平的研究证明，胃肠道无论是纵行肌还是环行肌均不具有发动SW的能力。慢波由存在于纵行肌和环行肌之间的ICC产生，这些细胞具有成纤维细胞和平滑肌细胞的特性，并与纵环两层形成缝隙连接，可将慢波传到平滑肌。近年的许多研究表明，ICC是肠道慢波的起搏者，在神经肌肉信号传递中起调节作用，而且与某些胃肠动力性疾病的发病有关。利尿钠肽natriureticpeptides家族是一类具有利尿、排钠，舒张血管，降低血压以及调节水盐和电解质平衡等作用的多肽。利尿钠肽家族的受体有3种，分别是A型、B型和C型利尿钠肽受体natriureticpeptidesreceptors，NPR。利尿钠肽通过与其受体结合而发挥作用。CNP作为利尿钠肽家族中的一员，是由Sudoh等人于1990年首先在猪的大脑中发现的，是一种具有22个氨基酸残基的肽类。CNP分布广泛，存在于中枢神经系统、心血管系统、泌尿生殖系统、消化系统以及皮肤和粘膜等，几乎遍布于整个机体。CNP作为一种局部调节因子通过自分泌和旁分泌的方式发挥作用，除了可以利尿、排钠，舒张血管和内脏平滑肌，降低血压以及调节水盐、电解质平衡以外，还可以抑制血管平滑肌的增殖调节肾素血管紧张素的活性和调节肠液和胆汁等消化液的分泌。此外还参与高血压、慢性心衰、慢性肾衰、动脉粥样硬化和肺纤维化等一系列病理过程的调节。CNP的许多作用是通过调节细胞膜上的离子通道，影响细胞内信号转导来实现的。我室以往的研究表明：在大鼠胃的各个部位都有利尿钠肽受体natriureticpeptidereceptor，NPR存在，并且在胃窦部最多；CNP可以抑制豚鼠胃窦部平滑肌的收缩活动，在此过程中细胞内cGMP生成明显增加；CNP通过cGMP-PKG途径明显增加平滑肌细胞钙激活钾通道电流。ICC是胃肠道平滑肌节律性运动的起搏细胞，直接影响胃肠运动的频率和幅度。然而，CNP对胃肠道平滑肌运动的抑制性调节是否通过对ICC起搏电流的影响来实现，国内外还没有研究报道。因此，本工作利用传统全细胞膜片钳技术在培养的小鼠胃和小肠ICC上记录起搏电流pacemakercurrent并观察了CNP对这些电流的影响。实验结果如下:1.培养24h后的ICC在倒置显微镜下观察可见：ICC已贴壁，成三角形或梭形。72h后可见ICC有梭形、三角形，核大，核周有少量胞浆。自核区胞体发出2～4条长突起，并与邻近细胞胞体及ICC突起相互连接成网络状。2.用c-kit免疫荧光染色后，在激光共聚焦显微镜或荧光显微镜下可以观察到，ICC表面酪氨酸激酶受体c-kit表达呈阳性，胞体和突起均明显着色。3.在传统的全细胞膜片钳模式，将膜电位钳制在-60mV的条件下，在体外培养的胃及小肠ICC上可以记录到自发而节律性内向电流，即起搏电流。胃窦部单一ICC的起搏电流振幅为130.78±9.59pA，频率为2.6±0.25次/分；胃窦部网络状ICC的起搏电流振幅为226.78±28.31pA，频率为3.6±0.51次/分；小肠部单一ICC的起搏电流振幅为79.65±11.23pA，频率为6.5±0.43次/分；小肠部网络状ICC的起搏电流振幅为272.32±38.84pA，频率为16.25±0.95次/分。在胃和小肠ICC之间以及单个ICC和ICC网络之间，电流幅度、频率有着明显的差异p＜0.05，n=6。4.在传统的全细胞膜片钳模式，将膜电位钳制在-60mV的条件下，通过增加电极内液中EGTA浓度达到10mmol?L-1使细胞内游离钙降低时，可以激活ICC的内向电流。5.钙调蛋白抑制剂对ICC起搏电流具有明显的激活作用。当细胞外给予W-750μmol?L-1时，起搏电流振幅由原来的48.18±1.34PA增加到137.175±11.54PA，增加了187.36±32.95%，钳制电流增加93.79±18.78PA。当W-7被洗脱后起搏电流幅度和钳制电流缓慢恢复。6.在激光共聚焦显微镜下免疫荧光染色的ICC细胞膜上利尿钠肽受本AnatriureticpeptidereceptorA，NPR-A和利尿钠肽受体BnatriureticpeptidereceptorB，NPR-B表达呈阳性，胞体和突起均明显着色，部分细胞发出长的凸起，并发现在小肠的表达明显多于胃窦部。<P对ICC起搏电流幅度具有明显的抑制作用，并具有剂量依赖性。当CNP浓度为0.01μmol?L-1，0.1μmol?L-1，1μmol?L-1时，分别使起搏电流抑制到对照组的76.06±1.51%；38.24±3.74%；29.60±1.93%。以上结果提示:1.小鼠胃和小肠ICC产生的起搏电流频率与小鼠胃和小肠自律运动的频率非常接近；网络ICC产生的起搏电流比单个ICC稳定、幅度高、频率快。2.ICC内游离钙浓度的降低是产生起搏电流的重要因素；钙调蛋白在ICC内参与起搏电流的抑制性调节。3.ICC细胞膜上存在心房肽受体NPR-A，NPR-B，在小肠中的表达大于胃。4.C型利尿钠肽通过ICC膜上的利钠尿肽受体对起搏电流产生抑制作用..……

Cajal间质细胞与胃肠道疾病
【摘要】：Cajal间质细胞(ICC)是胃肠道的起搏细胞,具有产生自发电信号、传导慢波电位、调节神经递质等功能。其形态数量的改变与多种胃肠道疾病相关,如贲门失弛缓症(AC)、胃食管反流(GERD)、术后胃瘫等。胃肠疾病中ICC的变化通常表现为数量减少或缺失,细胞突触变短,其形成的神经网络结构减少。胃肠疾病的发生部位、程度与ICC分布及数量关系密切,ICC为胃肠疾病的早期诊断、治疗带来了新的理念。
