Changing the Public-Private Hospital Mix

During the early 1980s, Greece introduced a major health reform, the main element of which was the attempt to establish a national health system. It coincided with similar efforts in other Mediterranean countries and with reforms in many European countries. The basic aims of health reform in European countries at the time were a universal right to health services for all citizens, a just geographical distribution of resources, and effective cost containment.
Health reform legislation in Greece did not includecost containment among its goals, but rather it put emphasis on the development of the public sector and on equity in health services consumption. In this respect it entered with a 15-year delay into what has been called the 'era of expansion and equity', characterising the orientation of health policy during the period from 1960 to 1975 in most developed countries.By the time health reform in Greece was fully implemented in 1986, most European countries were already experimenting with changing the roles of state and market in health care. While the pressure for change was uneven among the European countries, the trend regarding the role of government was common. Either through decentralisation and regionalisation, or through the introduction of some market elements, the effort was to reduce rigid central government controls and to strengthen the role of private initiatives and investment . The underlying objective was to improve system efficiency and reduce the burden on public resources. The health system in Greece was again moving in the opposite direction, roughly 15 years behind international developments. An overall assessment of the degree to which the 1983 health reform was able to meet its goals was attempted recently . In this paper we will only deal with the extent to which health reform achieved one of its main goals which was the full assumption of state responsibility for health services financing and provision and the severe limitation of the private sector. As we will show, these goals were not realised because of ineffective 'macro-management' of the system by the top political and administrative hierarchy, where health policy was designed and most of the important choices were made.
The Health Sector in the Early 1980s and the 1983 Health Reform: Twenty years ago the issue of health care reform was a major political issue in Greece. As rapid, deficit-financed, economic growth slowed down by 1973, and with the fall of the dictatorial regime in 1974, interest in matters of social policy became more pronounced. The pensions regime, traditionally sacred in Greece, was relatively unaffected, consuming the lion's share of public social expenditures . The health sector, on the contrary, characterised by public underfinancing and lack of a coherent policy, was in a state of total disarray, with a large degree of inequity and a deficit in the quantity and quality of health services offered to many segments of the population. With more than 700 small private hospitals mostly owned by doctors also working part-time in public hospitals, the private sector was very much present in all aspects of health care delivery, such as medical care offered by private physicians on a fee-for-service basis. As a reaction to a growing social problem, a working party set up in 1976 issued a report in which the major problems identified were the lack of infrastructure development in the hospital sector, provider eimbursement systems which encouraged inefficiency and induced demand,and lack of coordination among the multitude of social security funds. It was also the first time that the issue of private 'under-thetable' payments to providers was discussed at length. A follow-up on the 1976 Health Report was the Draft Health Bill of 1980 proposed by S. Doxiades, Minister of Health. The bill adopted many of the report proposals, adding partial establishment of the full-time exclusive employment for hospital doctors, a network of rural health centres, promotion of primary health care and public health measures, and upgrading of medical specialty training . This progressive health bill by a conservative government was a preemptive measure to an upcoming socialist party victory in the 1981 elections. Unfortunately, shortsighted opposition from inside the conservative party on charges of 'covert socialisation' did not allow the bill to reach Parliament. The socialists came to power in October 1981 with health reform high in their political agenda. In 1983, Law 1397 establishing the National Health System was voted in as a major legislative reform attempting a deep change in the financing and delivery of health services. The 1983 Law adopted the principles of universal coverage and equity in the distribution of services, to be financed by an increase of public expenditure from 4.5-5% of GDP. The major policy direction was towards strengthening the public sector in order to limit private expenditure and improve equity.
Results of the 1983 Health Reform:Assessment of the 1983 health reform is based mostly on data for the period . 1981 is the base period year, as some of the provisions of the 1983 health law were implemented by administrative measures in 1982. Conservative party governments in the period
relaxed some of the provisions, especially those concerning private sector investment activity. Data on health technology refer to the period after 1986, when the rate of diffusion of new technology and the requisite information increased.
According to OECD data, public social protectionion expenditures in Greece rose significantly from 10.9% of GDP in 1980 to 17.3% in 1993 . At the same time, Table 1 shows a significant increase in total health expenditures between 1981 and 1992, mostly a result of a considerable increase in private expenditure. At 3.6% of GDP, private health expenditure in 1992 was the largest percentage in the EU, and as Figure 1 shows, twice as important in terms of its participation in total health expenditure. Greece has always had high private expenditures which, in the past, exceeded public expenditures. What is surprising, however, is that the 1983 health reform failed to reduce private payments, despite the increase in public funding by almost 1% of GDP. The continued growth in private expenditure, as shown in Figure 2, is less due to payments for private hospitals, the bulk of it being out-of-pocket payments for physician services. Private pharmaceutical expenditure was kept stable because of rigid price controls and extensive coverage by social security, reduced by co-payments after 1992. Payments to hospitals and physicians, in turn, represent almost exclusively out-of-pocket payments to the private sector, since public hospitals and doctors are paid directly by social security. Table 2 shows the composition of private expenditure from the three household expenditure surveys conducted every 5 years.
Prospects for the Future:At the present time Greece faces another health reform. There are indications that some of the rigidities of the past will be abandoned, while the public sector still remains the major source of financing and delivery of health care. The only way, however, for the public sector to play its constitutional role as a guarantor of the efficient and equitable provision of adequate and high quality health services is a significant increase in productivity and perceived quality of care. Whether this will come to pass in Greece is the 'bet' of the next decade.
