目录1 拼音xuè qīng zhī fáng méi2 英文参考serum lipase3 概述(LPS)主要来源于,为一组专一性不高的酶,能水解多种含长链(8~18碳链)的,两端(α位)的脂肪酸较易被水解生成二酯,进一步又被水解为β甘油单酯。β甘油单酯很难被脂肪酶水解,常需先进一步变构为α甘油单酯。所以只有一小部分甘油三酯可以完全水解产生甘油。
此酶只于油和水的界面,速度和底物中颗粒有关。颗粒愈小,接触表大,反应速度快,而乳剂中底物浓度高低关系较小。因此,配制一个均匀而能长期底物乳剂是中一个很重要而又困难的问题,现在多用乳剂来配制底物。大致有、。
4 血清脂肪酶的医学检查4.1 检查名称血清脂肪酶4.2 分类 & 酶类测定4.3 取材血液4.4 血清脂肪酶的测定原理(1)滴定法:以橄榄油制成的乳剂为底物,血清脂肪酶催化其酯键水解,生成单酸甘油酯及脂肪酸,用标准脂肪酸,根据氢氧化钠用量计算LPS的活力单位。
(2)比浊法:甘油三酯与水制成的乳胶,因其胶束对入射光的及散射而具有乳浊。胶束中的甘油三酯在脂肪酶的作用下水解,使胶束分裂,浊度或光散射因而减低。减低的速率与脂肪酶活力有关。4.5 试剂(1)滴定法:
①20%(V/V)橄榄油乳剂:称取格替1.5g,置研钵中,研成细粒后,加入2g/L溶液5ml,继续研磨成糊状。再用2g/L苯甲酸钠溶液75ml,分次研钵,一并倒入捣碎机内,加入纯橄榄油20ml,初以8000转/min搅拌3min,继以高速(12000转/min)搅拌5min,即成乳白色的橄榄油乳剂。放冰箱保存。
②0.067mol/L,pH8.0盐:称取9.08g,用并稀释至1L,此为甲液。
称取无水9.47g,用蒸馏水溶解并稀释至1L,此为乙液。
取甲液5.3ml与乙液94.7ml混合,即为0.067mol/L pH8.0磷酸盐缓冲液。
③10g/L酞(酞)液。
④0.05mol/L氢氧化钠。
⑤95%乙醇或无水乙醇。
(2)比浊法:
①纯化橄榄油:称取纯氧化铝(70~325筛孔)10g,置于2mm×15cm层析柱中,使呈疏松及表面平整状态,缓缓加入级橄榄油18ml,用橡皮冲洗球加压,层析除去游离脂肪酸,得到无色透明的橄榄油约10ml,氧化铝转成黄色。
②橄榄油乙醇液:称取已处理的橄榄油2g,溶于无水乙醇中,加无水乙醇至200ml;
③Tris缓冲液(pH8.8):称取Tris 3g,去氧胆纳6g,溶于蒸馏水中,稀释到1L,用浓调pH至8.8;
④0.17μmol/L橄榄油乳剂:取500mlTris缓冲液,放入烧杯内,磁力搅拌器上,开动搅拌器,缓缓加入7.5ml橄榄油乙醇液(吸管尖端应插入液面以下),使呈均匀乳油液,冰箱保存。
4.6 操作方法(1)滴定法:按表1进行。
用0.05mol/L氢氧化钠滴定管和测定管,至呈持久的浅灰蓝色为止,记录滴定用去的氢氧化钠数。
(2)比浊法:
①于试管内加入4ml橄榄油乳剂,37℃预温5min,加入0.05ml,立即混合5次(切勿用力振摇!),迅速用分光光度计比浊,以Tris缓冲液调零点,在400nm波长读取吸光度为A1。
②将此管置于37℃水浴保温10min,然后同上读取吸光度为A2。
③如果很高,在保温期间,底物全部变清,则应将待测血清用Tris缓冲液作适当稀释新操作。
④标准曲线制作,取试管4只,分别加入橄榄油乳剂1.0、2.0、3.0及4.0ml,然后用Tris缓冲液补充到4.0ml,相当于甘油三酯浓度分别为0.17、0.34、0.51及0.68μmol,同上比浊,绘制标准曲线。4.7 正常值(1)滴定法:酶促反应4h为0.06~0.89U/ml,酶促反应16~24h为0.2~1.5U/ml。
(2)比浊法:呈正偏态,最低为OU,单侧95%上限为7.9U。4.8 化验结果临床意义胰腺是LPS最主要来源。血清LPS增高常见于及,偶见于。急性胰腺炎时,血清增加的时间较短,而血清LPS活性上升可持续10~15天。炎未累及胰腺时,LPS通常在正常范围。此外,总胆管结石或癌、、等有时亦可增高。4.9 附注(1)滴定法:
①原法加温24h,但后来发现在后16h内底物水解较少,而且操作时间太长,不能满足临床要求,故改为4h。
②橄榄油乳剂质量的优劣直接影响脂肪酶测定结果的准确性。乳剂乳化颗粒的大小及血清中脂肪酶是否能与乳化颗粒充分接触是重要因素。因此,每批制备的乳剂,其正常范围也略有出入,需用正常血清测试。
(2)比浊法:
①市售橄榄油必须用氧化铝处理,去除游离脂肪酸。使用未处理的橄榄油测出的脂肪酶活力只相当于处理过橄榄油测出结果的65%左右。
②约有5%~7%的正常人血清与底物保温后的吸光度比保温前略有增加,因而成负值。这可能由于血清中某些异常沉淀所致,例如含的常有此情况。遇有此种血清,可在血清中加入,使终浓度达55g/L,预孵10min,使(如)沉淀或解离,然后再用经此处理的标本操作即可。
